Massive äußere Blutungen können diesem Schema sogar als „c“ (critical bleeding) vorangestellt werden, da ein Verbluten noch vor dem Atemstillstand zum Tod führen kann.

    Bei Patienten mit einem Polytrauma wird häufig das Konzept der „Damage Control Surgery“ (Schadensbegrenzung) angewandt. Da der Körper durch den Unfall bereits massiv geschwächt, unterkühlt oder in seiner Blutgerinnung gestört ist, würde eine stundenlange, definitive Operation einen lebensgefährlichen zusätzlichen Stressfaktor darstellen, den sogenannten „Second Hit“. Stattdessen beschränken sich die Chirurgen in der ersten Phase nur auf das Nötigste – wie das Stoppen von Blutungen und das Verhindern von Infektionen –, um den Patienten schnellstmöglich auf die Intensivstation zur Stabilisierung zu bringen.

    Die tödliche Trias beschreibt einen Teufelskreis aus drei Faktoren, die das Überleben eines Schwerverletzten massiv gefährden: Unterkühlung, Übersäuerung des Blutes (Azidose) und Störungen der Blutgerinnung (Koagulopathie). Wenn die Körpertemperatur unter 35 Grad sinkt, arbeiten die Enzyme für die Blutgerinnung nicht mehr richtig, was zu unkontrollierbaren Blutungen führt. Das medizinische Team muss daher von Beginn an Wärmemanagement betreiben und gezielt Gerinnungsfaktoren verabreichen, um diesen biologischen Zusammenbruch zu verhindern.

    Normalerweise wird bei schweren Blutungen eine „permissive Hypotension“ angewandt, bei der man einen niedrigen Blutdruck bewusst zulässt, um frisch gebildete Gerinnsel an Wunden nicht wegzusprengen. Liegt jedoch ein Schädel-Hirn-Trauma vor, gilt das Gegenteil: Das Gehirn benötigt einen ausreichend hohen Blutdruck, um gegen den steigenden Druck im Inneren des Schädels anzukämpfen und weiterhin mit Sauerstoff versorgt zu werden. In diesem Fall hat die Durchblutung des Gehirns Vorrang vor der Zurückhaltung bei der Infusionstherapie.

    Obwohl Polytrauma-Patienten oft künstlich beatmet werden, empfiehlt die Leitlinie eine möglichst frühzeitige Ernährung über eine Magensonde (enteral), um die Darmbarriere aufrechtzuerhalten und das Immunsystem zu stärken. Ebenso wichtig ist die Frühmobilisation: Patienten werden oft schon nach wenigen Tagen passiv bewegt oder an die Bettkante gesetzt. Dies soll Muskelschwund, Gelenksteifigkeit und Infektionen wie Lungenentzündungen verhindern und den Grundstein für die spätere Rehabilitation und Rückkehr in den Alltag legen.
